Complete Your Patient Registration

If this is your first visit, please complete the Patient Registration Form below or with our receptionist upon arrival.

Completing your registration form ahead of time helps us prepare for your appointment and ensures we have accurate information for your medical records. The form collects your personal details, medical history, Medicare information, and emergency contact details.

Once registered, you can take advantage of our MyMedicare program for enhanced continuity of care. New patients are encouraged to book a standard 15-minute consultation, though longer appointments are available if you have multiple concerns to discuss.

For any questions about the registration process or to provide feedback, please contact us directly. Our practice manager is available to assist with any enquiries about becoming a patient at Madison Medical Practice.

Common Questions for New Patients

We highly recommend booking an appointment to see your preferred doctor. Our practice does not accommodate walk-ins, so please call ahead to ensure you are seen as soon as possible. You can book online or phone our reception team during business hours.

A standard GP consultation is 15 minutes. If you feel this is not enough time for your needs, please request a double consultation when booking your appointment. This is particularly useful if you have multiple health concerns to discuss.

On weekdays, bulk billing is available for patients aged 15 years and under, 70 years and over, or those who present a valid pension card before their consultation. Adults aged 16 to 69 are privately billed, with Medicare rebates claimable. For full details, visit our Fees page.

We are open Monday to Friday from 8:00 am to 6:00 pm, and Saturday from 8:00 am to 5:00 pm. The clinic is closed on Sundays. After-hours home GP services are available via 13 SICK (13 7425). Please note that Saturday consultations are privately billed for all patients.

Telehealth consultations are available for existing patients who have attended a face-to-face appointment within the last 12 months. For patients without Medicare, private fees will apply. Learn more about our telehealth options in our Practice Policies section.
